Enrollment Process

Thank you for choosing Sequoia Choice! Here are the steps to enroll at our school. You may contact the main office at 480-461-3222 if you have any questions about submitting your enrollment.

Please note, students in grades 3-8 and 10 will be required to participate in mandatory AzMerit and AIMS state testing in person at one of our scheduled testing sites. The test window for this spring is March 24 to April 7th. Students who enroll after April 7th, or who are in grades 1,2, 9, 11, 12, or Kindergarten will not be required to test during this Spring 2017 window.

2. Fill out an Enrollment Form


3. Submit the required documents.

  • In Person / Hand Delivered to a local academic center or the main office in Mesa
  • By Email to records@edkey.org
  • By Fax to 480 890-4106

Required for all students: Birth Certificate, Immunization Records, Proof of Residency.
Required if applicable: Special Education Records, Legal/Guardianship paperwork. An Unofficial High School Transcript is request to ensure a quick transition into online classes.


What to expect next...

Once you've submitted both the enrollment form AND the required documents, the school registrar will process the enrollment in the system, create student e-mail and network accounts, and assign your student to an advisor or elementary teacher. This person will contact you to provide information about starting school.

For IMMEDIATE ENROLLMENTS, within two school days of submitting the COMPLETE enrollment packet the academic coach will contact you to let you know the student is ready to begin.

For FUTURE ENROLLMENTS, the academic coach will contact you the day before the start date of that semester.

Grades K-6 – Your student's elementary teacher will CALL you on the primary phone number provided in the enrollment form.

Grades 7-12 – Your student's advisor will send an E-MAIL to the primary e-mail address provided in the enrollment form. Follow those instruction to get started.
